Forms for Breeders
I have several forms hanging on my kitchen wall that other breeders have seen and have requested copies of for their own use. After making printed copies for them, it occurred to me that other breeders might want copies also.
I have been using these for years. Each is "saved" so I can go in to change the year, make any important changes, and run new copies each January. These forms have been much used!
January 2011. Thanks to those of you who have written to tell me how much you appreciate having these forms. (One person doesn't breed Doxies!!!) Someone suggested I make changes to the top of the Immunization Chart so I have just listed the 3 important ones. (I still have all the specific "details" on my personal copy.)
WEIGHT CHART & WEIGHTS FOR MEDICATIONS
The DOB (second column) is for date of birth. Each year, I try to weigh each dog so I have a current weight.
IMMUNIZATION SCHEDULE
I use this to keep track of when Rabies shots and DHPP vaccinations are due. (Some of you may use a different vaccine combination.) In the Future column, I enter the future dates of both, which are then ready to be entered on the next year's form. It saves so much time! I don't have to go to each dog's folder every year to find all that information!

Critical!!! I have 2 copies of each of these forms---one in kitchen and one in Kennel Building. I refer these forms all the time!!!
